Circumstances of the Injury:
While stationed at his position, an artillery shelling began. Yurii was severely wounded by shrapnel in the head, chest, and limbs, causing him to lose consciousness. His comrades quickly administered first aid and evacuated him to safety.
Diagnosis:
Blast injury (April 30, 2024):
- Penetrating shrapnel gunshot wound to the head with a puncture fracture of the right parietal bone, moderate brain contusion, and multiple bone fragments embedded in the brain tissue.
- Multiple shrapnel wounds to the head and face, with small foreign bodies lodged in the soft tissues.
- Shrapnel wound to the posterior and lateral surface of the neck (right side).
- Shrapnel wound to the right side of the chest with right-sided hemopneumothorax.
- Shrapnel wound to the left hand and shoulder joint, with a gunshot fracture of the proximal part of the left humerus.
- Shrapnel wound to the left thigh.
Medical procedures performed (April 30 – May 1, 2024):
- Drainage of the right pleural cavity.
- Fasciotomy of the left lower leg.
- Surgical wound treatment.
- Resectional craniotomy of the parietal and occipital bones (right side).
- Removal of bone debris and foreign bodies from the right occipital area.
- Reconstruction of the skull with autologous tissue.
Treatment Plan:
Articulating spacer placement.
This procedure involves implanting a temporary device to replace the damaged joint. The spacer maintains minimal mobility and joint stability while preventing or treating infection and preparing the area for final reconstruction.
About Articulating Spacers:
An articulating spacer is a crucial step in managing severe combat injuries involving joints, particularly when there is a high risk of infection. In cases of bone and soft tissue damage caused by war-related trauma, infections are a common complication. Installing a permanent artificial joint immediately can lead to severe issues if the infection is not fully controlled.
The articulating spacer serves multiple functions:
- Infection Control:
The spacer releases antibiotics directly into the affected area, reducing the risk of infection. - Joint Structure Preservation:
It maintains the correct anatomical alignment and space between bones, preventing improper fusion or deformation. - Mobility:
Allows partial movement of the limb, reducing the risk of scar tissue formation and muscle atrophy.
Once the infection is eradicated and the tissues are prepared, a permanent joint prosthesis can be installed to restore full limb function.
