How he was injured:

He was at the company’s command post when another enemy airstrike hit—just another “routine” bombing with glide bombs, which had sadly become a common threat in their sector.

He had just relayed the alert about the incoming strike and made sure everyone who could was in cover. Then came the familiar sound—the whistling of an incoming shell. But this time, the explosion hit just a few meters from their shelter.

It was around 1:30 PM. The blast took out the electricity, and daylight suddenly filled the space. Everything was wrecked. At first, he noticed blood dripping from his head and nose. Then he realized something was seriously wrong with his leg.

He felt around and found a deep wound in the lower third of his left shin. When he moved it slightly, it was clear both shin bones were broken. He applied a tight bandage and then a tourniquet—there was bleeding, not heavy, but steady.

Once the skies cleared—no kamikaze drones overhead for the moment—his comrades and nearby units helped evacuate him to the roadside, where medics picked him up.

Diagnosis:
Gunshot and blast injury to the left lower leg with fractures to both the tibia and fibula in the lower third.

Explanation:
The patient sustained a severe gunshot/blast wound to the lower part of his left leg. The injury resulted in fractures of both major bones—the tibia and fibula—in the lower third of the leg.

This type of injury is especially complex due to several factors:

– The high-energy nature of the trauma (from an explosion or a high-velocity projectile)
– A high risk of infection because of the open wound
– Significant damage to the surrounding soft tissue
– A long and difficult recovery process, often requiring multiple surgeries and extensive rehabilitation

  • Clinical decision:
    Open reduction, bone defect reconstruction using autograft and bioactive glass, and internal fixation with a metal plate.

    Explanation:
    The patient underwent a complex surgical procedure that included:

    Open reduction: The fracture site was surgically opened, and the broken bone fragments were manually aligned into their correct anatomical position.
    Bone defect reconstruction using autograft and bioactive glass: A section of the patient’s own bone (autograft) was used, combined with bioactive glass—a modern biocompatible material that promotes new bone growth—to fill the area where bone was lost or severely damaged.
    Internal fixation with a metal plate: A specialized metal plate was used to secure the bones in place, ensuring stability and proper healing.

    Goals of the surgery:
    – Restore the bone’s integrity and shape
    – Securely fix the fractured segments
    – Stimulate natural bone regeneration
    – Preserve the function of the leg.
