Circumstances of Injury:

During the retreat from Vovchansk, Oleg and his comrades were caught under intense mortar fire. The force of an explosion threw him off a bridge, resulting in a severe injury — an open fracture of his leg.

Diagnosis:

Open Comminuted Fracture of Both Bones of the Right Tibia

Characteristics:

  • Open Fracture:
    • The fracture is exposed through the skin, increasing the risk of:
      • Infection: Immediate and rigorous wound care is essential.
      • Damage to surrounding soft tissues, blood vessels, or nerves.
  • Comminuted Fracture:
    • The bone is broken into multiple fragments, complicating the healing process.
    • May involve significant displacement of bone fragments.
  • Two-Bone Fracture:
    • Both the tibia (shinbone) and fibula (calf bone) are affected.
    • This disrupts the structural stability and weight-bearing function of the limb.

Clinical Decision:

Open Reduction and Internal Fixation (ORIF) of the Right Tibial Bones

Procedures:

  1. Open Reduction:
    • Surgical realignment of the fractured bone fragments.
    • Ensures accurate anatomical positioning to facilitate proper healing and restore function.
  2. Internal Fixation (ORIF):
    • Stabilization using metal implants such as:
      • Plates and screws
      • Intramedullary nail (IMN)
    • Provides:
      • Secure fixation to hold bone fragments in place during healing.
      • Restoration of the limb’s stability and ability to bear weight.
