Circumstances of Injury:

On August 6, 2023, while performing a combat mission, Mykola was stationed at his position when an intense mortar barrage began. One of the shells — a 120mm mortar round — hit the bunker directly. The explosion caused severe injuries to Mykola. Thanks to his comrades’ quick action, who applied tourniquets and stopped the bleeding, his condition was stabilized.

Due to the danger of movement during daylight hours, evacuation was delayed until nightfall. Mykola endured approximately four hours at the position. It was only at night that he could be safely transported to a medical point.

Diagnosis:

Explosive Trauma to the Left Lower Leg

Injury Details:

Caused by the impact of explosive fragments or blast force.
Likely involves:
- Soft tissues: Skin, muscles, blood vessels, and nerves.
- Bones: Fractures or defects.
- Joints: Potential dislocations or structural disruptions.
- Neurovascular bundle: Possible damage requiring careful monitoring.

Clinical Decision:

Removal of Metal Fixation Devices (MK) and Bone Defect Replacement with Bioactive Glass

1. Metal Fixation Device Removal (MK):

Metal implants (such as plates, screws, or other stabilizers) are removed after fulfilling their purpose of stabilizing the bone for proper healing.
Indications for removal:
- Achieved fracture stability.
- Infection at the implant site.
- Discomfort or chronic pain caused by the implant.

2. Bone Defect Replacement with Bioactive Glass:

Bioactive Glass: An advanced biomaterial that:
- Stimulates natural bone regeneration by providing a framework for new tissue growth.
- Possesses antimicrobial properties, significantly reducing the risk of infection.
- Integrates seamlessly with the surrounding bone or is gradually replaced by new bone tissue.

This approach not only restores structural integrity but also enhances healing while minimizing infection risks, ensuring Mykola’s successful recovery and return to functionality.
