Circumstances of Injury:
In May 2024, Oleksii sustained a severe injury during an assault on enemy positions. A nearby explosion severely damaged his left leg. Despite the shock, Oleksii continued his mission until he was evacuated to a hospital. Only there was the severity of his injury fully realized — fragments had shattered his left tibiotalar and ankle joints, necessitating extensive treatment and rehabilitation.
Diagnosis:
Post-Traumatic Arthrosis of the Left Tibiotalar and Ankle Joints (Grade III):
- Post-Traumatic Arthrosis:
- A chronic degenerative joint condition caused by prior trauma.
- Grade III represents the most severe stage, with nearly complete cartilage destruction, significant joint space narrowing, and deformation of joint surfaces.
- Tibiotalar and Ankle Joint Damage:
- These joints are critical for foot mobility and weight-bearing.
- Their severe impairment results in substantial walking difficulties and pain, even at rest.
- Persistent Pain Syndrome:
- Chronic and intense pain caused by joint destruction and inflammation.
- Significantly reduces quality of life and hampers everyday activities.
- Functional Impairment of Weight-Bearing and Walking:
- Restricted mobility due to pain and deformity.
- Inability to rely on the affected leg for support or normal movement.
Clinical Plan:
Arthrodesis with Resective Surgery and Intramedullary Rod Fixation:
- Arthrodesis with Resection:
- Surgical removal of damaged joint surfaces that no longer serve their functional purpose.
- The procedure immobilizes the joint by allowing the bones to fuse, alleviating pain and improving stability.
- Intramedullary Rod Fixation:
- An intramedullary rod is used for stable fixation of the fused bones.
- It provides a rigid structure that facilitates proper bone fusion and weight distribution.
