Circumstances of the Injury:
On October 1, 2022, Mykhailo and his team were defending the front line against an enemy breakthrough. During the battle, their combat vehicle was hit by a tank shell. Initially, Mykhailo didn’t realize the severity of his injuries due to the adrenaline masking the pain. It was only during evacuation that he noticed the extent of the damage to his arm. The searing pain left no doubt about the seriousness of his trauma. In that moment, Mykhailo feared he might lose his arm.
Diagnosis:
- Consequences of a blast injury (October 1, 2022) to the left forearm with a gunshot comminuted fracture of the left radius and ulna.
- Status post metal osteosynthesis (November 24, 2022) with plate fixation and intramedullary nail for fractures of both bones in the left forearm.
- Bone autografting (April 4, 2023) to address the defect in the ulna and radius, along with re-osteosynthesis of the radius.
- Consolidating fractures:
- Middle third of the radius.
- Upper and lower thirds of the ulna in the left forearm.
- Post-traumatic motor-sensory neuropathy of the left radial nerve, with moderate paresis of the left hand, sensory disturbances, and impaired motor function of the upper limb.
Clinical Plan:
Re-osteosynthesis of both bones in the left forearm with bone autografting.
Treatment Details:
- Re-osteosynthesis:
- A surgical procedure performed to stabilize and realign bone fragments in cases of nonunion or malunion fractures.
- Ensures correct anatomical positioning and enhances the functional recovery of the limb.
- Bone Autografting:
- Utilizes the patient’s own bone tissue, typically harvested from the iliac crest, to fill defects and stimulate bone regeneration.
- Promotes healing by integrating seamlessly with the existing bone, reducing the risk of rejection or complications.
