Injury Circumstances:
At midnight, Ivan’s unit was visited by comrades from another division. Together, they were preparing for the next day, but at 3 a.m., the enemy launched an attack with two missiles, destroying the building where they were gathered. The explosion was so powerful that it mixed everything around with fragments of reinforced concrete.
The blast threw Ivan into the basement under the collapsed structure. Feeling sharp pain in his leg, he realized something was wrong: his leg was shorter, and his foot was lying nearby, attached only by tendons. Upon touching, he felt exposed bone, but despite the pain and shock, he managed to apply a tourniquet himself.
Summoning his remaining strength, Ivan crawled out from under the rubble. In the evacuation vehicle, it was unclear whether his leg could be saved. Ahead lies a series of surgeries and a long recovery period to restore the limb’s function.
Diagnosis:
Open fracture of the external and internal malleolus of the left lower leg with displacement.
This injury involves fractures of both the fibular and tibial malleoli, with the bone fragments displaced from their normal position. The fracture is accompanied by an open wound, increasing the risk of infection. Such an injury compromises the stability and function of the joint, requiring surgical intervention to clean the wound, restore the position of the fragments, fix them, and follow up with treatment to regain leg function.
Clinical Decision:
Open reduction of fractures, metal osteosynthesis with plates and screws.
This means that during the surgery, the bone fragments will be surgically aligned into their correct positions (reduction). Metal plates and screws will be used to fix the fragments, providing stability and promoting proper healing of the bones. This method helps restore the correct anatomical structure of the lower leg and reduces the risk of complications, such as improper bone healing or joint instability.
